3. Representative?s Acknowledgment Not To Engage In Unethical Sales Practices
4. Representative?s Obligations Relating To The Sale Of Long Distance Services
5. Sign Up Form

Enter your Stores Code (Form should be CDXXX) obtain this from your Store Manager.
